The Elderly and Disabled Waiver is a statewide program administered and operated by the Mississippi Division of Medicaid (DOM). It is designed to offer assistance and services in a home or community-based setting if you qualify for Medicaid. The program allows you to remain in a home or community-based setting as an alternative to nursing facility care or other institutional care.
To qualify for the Elderly and Disabled Waiver, you must meet the following eligibility criteria:
Upon approval of home and community based-services, the following services are offered through this waiver by HamiltonDavis Home Care. Inc.:
Non-medical Assessments are conducted to find clients’ functioning capabilities, to identify their needs, and to determine what services are required to assist them attain and maintain optimum independence. HamiltonDavis Homecare Agency, Inc. will identify the following needs which are not being met and provide services to individuals in their own homes and communities, who need assistance caring for themselves because of old age, sickness, disability and/or other inflictions. We will visit clients in their home environment to assess their service needs, wishes and preferences and to determine if the Agency has the skills and competence required to meet their needs. The life-enriching services we provide help the disabled throughout the community to preserve an independent and dignified lifestyle while in the comfort of their own homes. The assessors may review subsidization possibilities, develop Care Plans, make referrals, and arrange for Service Delivery Schedules according to the clients’ wishes.
